Forty new charts, logs, and other medical forms have been added to the website FreePrintableMedicalForms.com.

The new medical forms include printables for medical offices, school nurses, home health care providers and anyone managing their own health, fitness, or diet.

Just-added medical charts include: an anesthesia record, blood count results, daily vitals flow sheet, a fluid balance chart, a medication flow sheet and a patient injury tracker.

There are new journals and logs such as a controlled substance usage log, an elimination diet tracker, a food reintroduction symptom tracker, a medication reaction tracker and a nebulizer treatment log. The wallet cards category now has printables alerting people to conditions such as aphasia, dementia, and epilepsy as well as allergies and general medical information.

FreePrintableMedicalForms.com also now has: an alcohol questionnaire, an allergy action plan, an asthma action plan, birth plans, a first aid treatment form, medication forms, a medication incident report, a pre-employment physical form and a school medication administration log.
